[boost] [Parameter] Regression test failures

The change applied to Trunk for Ticket #1535 has caused a bunch of regression
test failures in the Parameter library, due to some missing preprocessor
includes.

I have opened Ticket #1541 for this, and attached a patch which allows the tests
to pass on MSVC 7.1/8.0/9.0.
